Useful Information and Death Notices

Obtaining a death certificate when a loved one passes away in one of the following places:

Nursing Home/Hospital/Hospice

One can obtain a death notification form from a nursing home/hospital or hospice. This form has the cause of death. Once you have received this, make an appointment with your local births/deaths/marriages office. Bring the death notification form with you and your ID and a death certificate will be issued to you.

At Home when the deceased has been under doctor’s care

One can obtain a death notification form from the deceased’s doctor. This form has the cause of death. Once you have received this, make an appointment with your local births/deaths/marriages office. Bring the death notification form with you and your ID and a death certificate will be issued to you.

Sudden Death

When someone dies unexpectedly, the coroner will be involved. A post mortem is normally required and the deceased will be brought to UHL Mortuary where an examination of the deceased is carried out to determine the cause of death. In this situation, the funeral may be delayed by a day or more.
